Body Mass Index (BMI) is a quick screening number used by clinicians worldwide. It's calculated from your height and weight alone — no blood test, no equipment needed. Here's what the formula is, what your number means, and where BMI falls short.

1. The BMI Formula

Both formulas give the same result — 703 is the unit conversion factor from lbs/in² to kg/m². Height must be squared, which is why a small difference in height has an outsized effect on BMI.

2. BMI Categories (CDC / WHO)

Below 18.5 Underweight May indicate nutritional deficiency; increased risk for some conditions
18.5 – 24.9 Normal weight Associated with lowest health risk in population studies
25.0 – 29.9 Overweight Slightly elevated risk for cardiovascular disease, type 2 diabetes
30.0 – 34.9 Obese (Class I) Moderate elevated risk; clinical intervention often recommended
35.0 – 39.9 Obese (Class II) High risk; weight loss typically recommended by clinicians
40.0 and above Obese (Class III) Very high risk; may qualify for bariatric interventions

3. Worked Example

Person: 5 ft 10 in tall (70 inches), weighs 185 lbs

Step 1 — Convert height to inches: 5×12 + 10 = 70 inches

Step 2 — Square the height: 70 × 70 = 4,900 in²

Step 3 — Apply the formula:

BMI = (185 ÷ 4,900) × 703 = 0.03776 × 703 = 26.5

Result: BMI of 26.5 falls in the Overweight range (25–29.9).

4. Limitations of BMI

BMI is a useful screening tool but a poor diagnostic tool. Major limitations:

Muscle vs. fat
Muscle is denser than fat. A 200-lb athlete with 10% body fat and a sedentary person with 35% body fat can have the same BMI. BMI cannot distinguish between lean mass and fat mass.

Age
Older adults naturally lose muscle mass (sarcopenia) and gain fat even if weight is stable. A normal BMI in a 70-year-old may mask unhealthy body composition.
🌍
Ethnicity
Asian populations have higher cardiovascular risk at lower BMIs. The WHO recommends lower thresholds for some Asian groups (overweight at 23+, obese at 27.5+).

Sex differences
Women naturally carry more body fat than men at the same BMI. A BMI of 25 represents different health risks for men vs. women.

Height extremes
Very tall people tend to have lower BMIs than accurate body fat would predict; very short people tend to have higher BMIs.

Special populations
Not applicable to pregnant or breastfeeding women, elite athletes, children (use BMI-for-age percentiles instead), or bodybuilders.

5. BMI for Children and Teens

For children ages 2–19, BMI is interpreted differently — using BMI-for-age percentiles that account for normal growth curves. The same BMI number means different things at different ages:

Children's BMI percentiles are calculated using the CDC's age- and sex-specific growth charts. Always consult a pediatrician for children's weight concerns.

6. Alternatives to BMI

When BMI doesn't tell the full story, clinicians use these complementary metrics:

  • Waist circumference — abdominal fat is a stronger predictor of cardiovascular risk than total weight; risk elevated at >35 in (women) / >40 in (men)
  • Waist-to-hip ratio (WHR) — compares waist and hip measurements; >0.85 (women) or >0.90 (men) indicates elevated metabolic risk
  • Body fat percentage — measured via DEXA scan, bioelectrical impedance, or skinfold calipers; directly measures fat vs. lean tissue
  • Waist-to-height ratio (WHtR) — simple rule: waist circumference should be less than half your height; correlates well with cardiometabolic risk
  • Visceral fat index — some body composition scales and clinical imaging can estimate visceral (organ-surrounding) fat, which is more metabolically active than subcutaneous fat
